IMPORTANT NOTES:

Vaccines are given by appointment only – you MUST be registered.

As the slots in each of the above dates are filled, the link will shut down.

Once you have an appointment, you are guaranteed a vaccine.  Please do not arrive at the Middle School more than 15 minutes before your scheduled time.

You will need photo identification (with birthdate) to receive your vaccine.

There will be volunteers to assist you in the process from the time you enter the parking lot until the time you leave the facility.

Once you have received your vaccine, you will be asked to stay in the observation area (inside the building) for an additional 15-30 minutes.

You will be registered for your second dose of the vaccine while at the clinic – this dose will be given exactly 4 weeks later, again at the Burrillville Middle school. Do not register if you are not available for the 2nd dose four weeks later.

You will be receiving the MODERNA vaccine.
